Add a Form Designer module to my public Web site:
On the left navigation bar, click Web Site.
In Page Manager, (next to the name of the Web page) containing the list to be published,
Click Edit.
In the toolbar of Page Editor,
- Click Module,
- Click Form Designer.
In the App and List boxes,
- Choose the app that I want
- Select the list that I want my form to connect to.
